Initial Visit
Please bring previous x-rays and reports with you, if you have them, for your initial visit. Also, wear loose fitting clothing if possible. The initial visit lasts about 1 hour and future treatments take about 15 minutes to 30 minutes.
Please also print and complete the two Patient Entrance Forms (see below) and bring them with you the first time I see you. This will allow you to fill out these forms at your leisure.
